As a recipient of a Community Foundation Scholarship, I agree to provide the Community Foundation of Greater Chattanooga, Inc., with the following:

1. A copy of my grade report at the end of each grading period.
2. An original official transcript at the end of each academic year.
3. Notification of any changes so that my file may be kept up to date.
   Such changes would include:
      - Discontinuation of education
      - School transfer
      - Change in home or school address
      - Change in telephone number
4. Failure to provide the Community Foundation with a copy of my grades at the end of each semester will result in loss and/or nonpayment of the scholarship.
